1.) I Acknowledge... *

I understand that all membership changes must be made at least 3 days before my next billing date. Anything after will be applied to the following month.

2.) I Acknowledge... *

I understand that IF I am currently in a 6 month or 1 year contract, my account will be drafted a $300 Early Termination fee upon canceling. This fee DOES NOT apply to kids memberships as they are NOT in contract.

3.) I Acknowledge... *

I understand that IF I am no longer in contract and currently receiving Discounted Monthly Rate, that discounted rate will No longer apply if/when I choose to return at a later date. I understand that I will need to start a new contract at Current Gym Rates upon returning.

4.) I Acknowledge... *

I understand that by canceling my membership, I will no longer have access to any Solus training and accountability tools including viewing workouts through SugarWOD, accessing Home workouts, our private members Facebook group or our class Group Me’s.
