I understand that I must provide 28 days notice to cancel my membership at CrossFit Hurstville.

I understand that my membership will be cancelled 28 days after the receipt of my cancellation request.

I understand that any membership fees scheduled to process within the 28 days will process as normal.

I understand there are no refunds for membership payments that occur within the notice period.

I give permission to CrossFit Hurstville to charge the $200 cancellation fee to my nominated account if this is in the first 3 months of my membership at CrossFit Hurstville.
